Each Hub consultation team includes a child and adolescent psychiatrist(s), behavioral health clinician(s), program coordinator and a family peer specialist.<\/span><br><\/p>\n\n\n\n<div style=\"height:21px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p style=\"font-size:22px\"><strong>Services:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Free Telephone Consultation Within 30 Minutes of Initial Call<\/li><li>Assistance With Finding Community Behavioral Health Services<\/li><li>Behavioral Health Training and Education<\/li><\/ul>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p style=\"font-size:22px\"><strong>Consultations result in one of the following outcomes depending on the needs of the child and their family:<\/strong><\/p>\n\n\n\n<ol class=\"wp-block-list\"><li>An answer to the PCP&#8217;s question<\/li><li>Referral to the team\u2019s program coordinator or family peer specialist to assist the family in accessing local behavioral health services<\/li><li>Referral to the team\u2019s behavioral health clinician to provide transitional (interim) face-to-face or telephonic support to the child and family<\/li><li>Referral to the team\u2019s child psychiatrist for a face-to-face diagnostic evaluation or psychopharmacological consultation<\/li><\/ol>\n\n\n\n<div style=\"height:56px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p style=\"font-size:22px\"><strong>Contact Us:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Hartford Hospital Hub Team: 855-561-7135<\/li><li>Wheeler Clinic Hub Team: 855-631-9835<\/li><li>Yale Child Study Hub Team: 844-751-8955<\/li><\/ul>\n\n\n\n<div style=\"height:24px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n","protected":false},"excerpt":{"rendered":"<p>Who We Are: The ACCESS Mental Health for Youth program consists of expert pediatric psychiatry consultation teams located throughout the state of Connecticut to help PCPs meet the needs of children and adolescents with mental health problems.
